To evaluate whole-body MRI (WB-MRI) as an alternative approach for the staging of patients with stage IV Hodgkin lymphoma (HL) and to compare the results with those of 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET)/CT and PET/CT-lymphoscintigraphy. This study included 24 patients with histologically confirmed stage IV HL, who received WB-MRI up to 4 days before their lymphoscintigraphy and PET/CT; these procedures were primarily used to confirm the extent of their HL. WB-MRI findings were compared with clinical and pathological findings, and concordance with PET/CT and lymphoscintigraphy was analyzed. Of the 24 patients studied, 14 had primary HL, 8 had recurrences, 1 had mixed-cell HL, and 1 had blow up of an HL nodule. Concordance of the WB-MRI results with pathological findings was null for 10 patients, 2 had a borderline discordance, and 12 had a concordance. All WB-MRI, lymphoscintigraphy, and PET/CT findings were confirmed by histology or clinical evolution. The P values for the comparison between WB-MRI and 18F-FDG PET/CT results were: 0.34 and 0.11, respectively, and for the comparison between WB-MRI and lymphoscintigraphy results were: 0.59 and 0.34, respectively. The findings on WB-MRI are concordant with those on pathological examination and PET/CT in patients with stage IV HL.
