To reduce observer variability, proportion of TDLU within the biopsy specimen was calculated using an interactive digital imaging and analysis system. The margins of a biopsy are typically marked with a pencil to ensure the exact positioning of subsequent sections. In the present investigation, images from the right breast were viewed, and the approximate location was identified relative to a set of landmarks on the skin. Digital images were calibrated to produce accurate size measurements. Using a ruler, TDLU were measured from the inside of the inner margin to the inside of the outer margin of the biopsy. TDLU were scored as a percentage of the total area of biopsy, that is, the percentage of the total biopsy occupied by TDLU. Volume data were used to approximate results based on the formula: V=¼π×(D/2)2×2×T, where V is the volume of TDLU, D is the mean TDL diameter (1,000×D/2), and T is the mean TDLU thickness (1,000×T/2). This formula uses the Pythagorean theorem of the right triangle. The measures and calculations were performed by an independent, calibrated reader who was blinded to pathology results and procedure landmarks. For the final analysis, the numerical data were analyzed using the ANOVA model (SD/mean). The mean TDLU volume for the patients in this study was 88,256 microns (1 micron=1/1,000th of a millimeter or 0.001 mm).

HR-MRI used the GE Signa 3-T MRI system. The scanner operates at a field strength of 3-T (3 teslas), which is 30% stronger than the 1.5-T machine described above; this strength allows images to be acquired in about twice the amount of time, which is important for minimizing patient discomfort and fatigue. T1-weighted MRI pulses are used along with a fat-suppressed T2-weighted pulse during the first phase of each MR acquisition when there is no motion. This greatly helps reduce artifacts and improve diagnostic accuracy by shortening the overall time of the study. The contrast agent is distributed throughout the blood pool. The common, strong capillary vessels around the breast tissue are captured by the fat-suppressed sequence, which is done by inversion and saturation pulses. This produces a unique image, which can be obtained immediately after the enhancement is delivered with the administration of the contrast agent.
